Community Access
Encouragement to participate in appointments, outings, and meaningful activities in the community.

Transportation Help
Support with transportation coordination for appointments, errands, and approved activities.
Family Partnership
Ongoing communication with families, coordinators, and referral partners to support continuity of care.
