Admissions
Supportive Admissions Process
Gateway Support Services helps families, guardians, and referral partners explore Community Living Arrangement options with a clear, compassionate, and individualized admissions process for adults in Georgia.
How admissions works
Our team works closely with families and professional referral sources to understand support needs, review fit, and plan a smooth transition into a safe and structured residential setting.
01
Initial inquiry
Call or email our team to discuss the individualโs needs, goals, current supports, and preferred timeline.
02
Assessment review
We review available records, support plans, behavioral considerations, and daily living needs to determine appropriate placement.
Our intake process is responsive and focused on individualized assessments, collaboration with support teams, and a commitment to quality care.
03
Visit and planning
When appropriate, we coordinate a conversation or visit to help everyone understand the home environment, routines, and expectations.
04
Transition support
Once approved, we help coordinate next steps for move-in, communication, and ongoing partnership with the care team.

What to prepare
Helpful admission details
Having the right information ready helps our team evaluate needs and respond efficiently. If some items are not yet available, we can still begin the conversation.
Referral information
Share contact details for the individual, guardian, family member, or referring professional along with the reason for referral.
Clinical and support records
Relevant assessments, diagnoses, medication information, behavioral supports, and current service plans help us understand the full picture.
Daily living needs
Tell us about mobility, personal care, communication style, routines, transportation needs, and areas where support is needed most.
Goals and preferences
We want to understand strengths, interests, community goals, and what a successful residential placement looks like for the individual.
